The Washington Post has a video report which shows the ride breaking apart. I wouldn't recommend watching it. That report says the ride passed inspection.
It sounds like metal fatigue. The witness at the end of the WP video says seats in the section that came off were shaking badly right before the accident.
I once had a restraint bar break completely off mid-ride, and I had to hold the bar with one hand to keep it from falling down and killing someone and holding on for dear life with the other.
One high school summer I "threw" and "tore down" much milder rides for a traveling show. Wouldn't trust my old crew to flawlessly build and breakdown that machine many times a year.

The cause could be imporoper assembly, but I wonder if the ride wasn't engineered for the passenger weight.

Given how old some of these rides are, that's an interesting point.
Including the accident it is still safer to ride the rides than drive drunk and on heroin while receiving oral (paid-for) pleasure on the way there...

I only text fresh selfies, the safest texts there are, so I have determined texting improves my driving by about a half-joint's worth.
