Reagan is remembered by millions of Americans as a great and beloved President, and Trump knows that, but I am sure he also knows that the anti-Trumpers will leap at the opportunity to say, but Reagan was senile and that's what we think you are too, ha ha.
I presume Trump not only knows that he is provoking that reaction but knows that reaction will backfire. It will backfire because: 1. Anyone who likes Reagan already rejects the idea that he was impaired by Alzheimer's Disease while he was President, 2. It's offensive to appropriate the misfortune of Alzheimer's Disease for making cheap political points, and 3. Many people have experienced immense pain dealing with Alzheimer's Disease, and that emotion can only be reshaped into a reason to hate Trump if you trample on that pain.

The totally screwed up US medical system affects everywhere in the US, it is extremely costly, vs European standards, in every state. More in some than in others, but this is a national problem. It bloats private insurance costs even more than Medicare/Medicaid. And its been doing so for decades, the divergence vs Europe began, IIRC, back in the 1970s.
No matter how much people want to be let alone, they aren't being let alone. More, they may not realize how much they are being imposed upon. Taxation is sneaky. Often it takes the form of regulations that impose costs, transferring money to favored parties, without passing through the government. This is one reason why Euro-US comparisons vis "socialism" are very murky.
People have a hard time seeing out of their bubbles. It takes experience and an active curiosity to cross through.

Interesting question, and worth exploring. A related, and more interesting question is the difference between any modern political system and fascism. The little problem here is that they, "reactionary" conservatism of a European flavor, fascism, "socialism", Euro or third-world variety, etc., are all born of the same roots, the 19th century reaction against big-L Liberalism and big-S Socialism. These are all "third way" systems that in one way or another attempt to split the difference. And in the modern world this is the default political compromise.
We are all Fascists now.
